RE: UPDATED INFORMATION REGARDING THE CENTERS FOR MEDICARE AND MEDICAID SERVICES (CMS) ESTABLISHMENT OF A BENEFICIARY AND FAMILY CENTERED CARE NATIONAL COORDINATING CENTER
IPRO CONTACT: Andrea Goldstein, Vice President, Medicare/Federal Health Care Assessment, 516-209-5364
The purpose of this memorandum is to make you aware that, effective Monday, February 6, 2012, the intake process (including medical record requests) for Medicare beneficiary quality of care complaints will transition back to the local QIO in the state or jurisdiction in which the care occurred. As you may recall, in November 2011, IPRO issued Medicare Administrative Memorandum #2011-03, which described the changes made by the Centers for Medicare and Medicaid Services (CMS) to the QIO intake process, including designation of FMQAI as the centralized intake unit. However, in light of this CMS change and the reversal of the intake process back to IPRO, any recent or outstanding requests for medical records that you may have received from FMQAI should be sent directly to IPRO instead.
Requested medical records may be faxed to IPRO at the following fax server number: 516-326-6119 or mailed to IPRO at the following address:
Medicare/Federal Health Care Assessment
1979 Marcus Avenue – 1st Floor
Lake Success, NY 11042-1002
Attention: Beneficiary and Family Centered Care
Please include a copy of the FMQAI Medical Record request form with the medical record.
Should you have any questions in regard to medical record requests from either FMQAI or IPRO, please call Elizabeth Hall, Senior Director at 516-209-5394.
Effective February 6, 2012, IPRO will resume direct communication with IPRO Liaisons in regard to medical records required for review as we have done in the past.
Should you have any questions in regard to this Memorandum, please feel free to contact Andrea Goldstein, Vice President, Medicare/Federal Health Care Assessment.
